Yes. We require a copy of your most recent insurance card(s) each time you are present for service to ensure that we are billing the correct insurance company. Insurance can change from month to month, so it is important that we have a copy for correct claims filing information.
Account Services will make every effort to resolve your account with your insurance company. Occasionally, we will be unable to resolve the issue with your carrier and will need your assistance.
A co-payment is a set fee the member pays to providers at the time services are rendered. The patient should contact their insurance company to ask about co-payment amounts prior to the date of service.Â
Co-insurance is a form of cost-sharing. After your deductible has been met, the plan will begin paying a percentage of your bills. The remaining amount, known as co-insurance, is the portion due by the patient. You can get details regarding your co-insurance, if applicable, from your insurance company.

Yes, we will submit claims to secondary insurance after your primary insurance has processed your claims.
This depends on the insurance coverage, insurance providers are required to pay for the COVID test portion, and some insurance companies are covering the test and the full office visit. Other insurance companies are leaving a portion of the copay or coinsurance to the patient’s responsibility. It’s best to reach out to your insurance company before your visit to confirm what your coverage will be.
If you are asymptomatic and have not been directly exposed to COVID-19, your insurance plan is not required to cover the cost of testing or services for screening purposes. These testing services are available at our office for a flat rate fee and do not require an office visit. See pricing here.
